On April 24, 2018, the United States Citizenship and Immigration Services (USCIS) announced that the USCIS Phoenix Lockbox Facility is now accepting the Form N-565, Application for Replacement Naturalization/Citizenship Document. The Nebraska Service Center ceased accepting the Form N-565 on April 18, 2018, and now forwards all Form N-565 applications to the Phoenix Lockbox.
The USCIS’s decision to move Form N-565 processing to the USCIS Phoenix Lockbox Facility means that applicants may now pay the filing fee by credit card using the Form G-1450, Authorization for Credit Card Transactions. The USCIS had announced on February 14, 2018, that the credit card payment option would be available for 41 USCIS fee-based forms that are processed at USCIS lockbox facilities. The Form N-565 is a form that is used to apply for a replacement Declaration of Intention; Naturalization Certificate; Certificate of Citizenship; Repatriation Certificate; or to apply for a special certificate of naturalization as a U.S. citizen to be recognized by a foreign country.
When filing the Form N-565 or any other form, it is important to carefully follow the form instructions from filling out the form to properly filing it with the requisite filing fee.
